If this is your first appointment we ask for you to call in so that we can receive and relay information about the visit. Future appointments you may schedule via schedule link, phone, text and email. We offer PDF fillable or printable links for Addendum Contract and Health Intake.
Missed Appointment or Late Cancellation Policy
Any missed appointment or late cancellation of 6 hours or less will be charged a 50% of appointment fee. Please note insurance cannot be billed for missed appointment or late cancellation fee.
